SDOH Pannel Discussion: There is increasing recognition that addressing social drivers of health such as food insecurity, housing, transportation, and social isolation can improve outcomes for patients, lower the total cost of care, and help advance health equity. Emerging mandates from CMMS and NCQA will also push providers and payers to do more to screen and identify social barriers to health for patients and members, and ultimately help address those issues. In this session you will hear directly from two providers who have been working to integrate addressing social drivers of health into how they practice whole person care, discuss the critical intersection of behavioral health and social care, and learn some tips and best practices for becoming a social determinants of health champion in your own practice.

FUM Q&A:
Developed by the National Committee for Quality Assurance (NCQA), FUM assesses the efficiency with which healthcare providers monitor patients post-discharge from mental illness-related visits to the emergency room. The core objective of the measure is to optimize patient care by ensuring timely follow-ups, with a focus on visits within a week or up to 30 days after Emergency Department (ED) discharges. By emphasizing the importance of these follow-ups, the healthcare community can potentially reduce repeat ED visits, enhance patients' physical and mental well-being, and ensure that patients consistently adhere to after care instructions that will aid in their recovery. During the session, the team will elaborate on best practices for healthcare professionals to bolster follow-up, emphasizing the pivotal role of clear communication between care providers, timely scheduling, and the provision of essential paperwork. Real-world scenarios, like patients visiting ED for symptoms that mask underlying mental health concerns, will be discussed. Additionally, the team will walk through the technical specifications of the FUM measure, detailing the inclusion and exclusion criteria, and common misconceptions. The seminar will conclude with frequently asked questions to address any outstanding queries related to the FUM measure.
